Associations/Independent Teams Approved for 2011-2012

The following documentation must be submitted to the CAHA Secretary/Treasurer prior to being cleared to roster teams for the 2011-2012 Season.  All documents must be included in the submission - partial submission will not be processed.

  • Bylaws
  • Policies and Procedures
  • Board Members (Points of Contact) in CAHA provided format
  • Affiliate Agreement
  • CHA Annual Documentation Checklist - Completed

Cleared Associations and/or Independent Teams will be posted when ALL required documents are received and the reveiw is completed.
