CAHA Documents

The following documents must be provided to the CAHA Secretary/Treasurer prior to teams being registered for the 2011-2012 Season:

• CAHA Association Document Checklist  (Click on link below to downlaod checklist)
• Bylaws
• Policies and Procedures
• Points of Contact in CAHA provided format
• Affiliate Agreement (Click on link below to download document)

NOTE:  Assocations may combine bylaws and policies/procedures into one document as long as all areas on the CAHA Association Document Checklist are addressed.

All required documents must be submitted as a package - Partial submissions will not be processed.
