Physicals
All Bishop Canevin High School athletes ( 9th thru 12th ) need a signed PIAA Physical before they are allowed to participate in school sports. Use the link below to download your PIAA Physical form. If you plan on participating in ANY sport at Bishop Canevin, you MUST HAVE a completed physical turned into the school.  Bring your completed forms to the first practice and give to your coach or send to the school nurse.

Please keep a copy of your forms as backup.  NO PLAYER is allowed to practice without having turned in a physical.
Impact Baseline Testing

Impact Baseline Testing – Canevin’s policy is that students should have an impact test every two years.  Therefore freshman, juniors and all transfer students who are playing a sport are required to have a current impact baseline concussion test.  If you are a sophomore or senior and have not had an impact test in two years you are also required.

Your pediatrician should be able to help guide you to a local testing area to have the testing completed.  Usually there is a $25-$30 fee for private physician testing.

they can represent the school at any game.
Player Documentation

Each player must complete the following forms which enable us to register you on the Bishop Canevin team.  Please bring the following forms to the first practice and give to Paula Krivak.

Copy of Birth Certificate:  If you did not play on the Bishop Canevin team in 2013-14 we will need a copy of your birth certificate.

USA Hockey Consent to Treat form, signed by the parent/legal guardian.

US Hockey confirmation number 2014-15 season.
       If you don't know your number you can access it at www.usahockeyregistration.com , click on 'Request Duplicate Registration Confirmation' or contact USA Hockey at 1-800-566-3288 ext 123.  All players must be registered with USA Hockey.

All transfer students and incoming freshman have special forms to fill out.  Check with our registrar Paula Krivak as to what information is needed.
