- 2017/18 USA Hockey Registration number (https://usahockeyregistration.com/login_input.action)
- Birth certificate (if you are new to Hampton Ice Hockey)
- Signature page from the Player and Parent Handbook signed by both player and parent (download the handbook below)
- Completed Player Registration Form and fees (download the Form below)
- Signed Consent to Treat (download the form below)
- Completed Participant Code of Conduct (download the form below)
