Team/Player Waiver Form

All players and staff members of participating teams are required to complete the waiver form before their first tournament game.

Please read the waiver carefully, all fields at the bottom of the form are required to be filled in.

If the player is under the age of 18, the form MUST be signed off by a parent or guardian.

In consideration of being allowed to participate in any way in the TEP TOURNAMENT, Toronto, Ontario related events and activities. I acknowledge, appreciate and agree that:

1. The risk of injury from the activities involved in this program are significant, including potential permanent paralysis and death, and while particular rules, equipment, and personal discipline may reduce the risk, the risk of serious injury does still exists; and

2. I knowingly and freely assume all such risks, both known and unknown risks, even if arising from the negligence of the releasees or other, and assume full responsibility in participating; and

3. I willing agree to comply with the stated and customary terms and
conditions of participation. If however I observe any unusual significant hazards during my presence and my child’s participation, I will remove my child from participation and bring such to the attention of the nearest tournament official immediately; and

4. I, for myself, and on behalf of my heirs, assigns, personal representatives and next of kin, hereby release and hold harmless TEP Hockey, their officers, officials, agents and/or employees, other participants, sponsoring agencies, sponsors, advertisers, and if applicable owners and lessors of the premised used to conduct the event (“releasees”), with respect to any and all injuries (Including Concussive or Neurodegenerative Injuries), disability, death, or loss or damage to person or property, whether arising from the negligence of the releases or otherwise.

5. I understand that the TEP Tournament has rules, noted under "Helmet Rules", and do agree to wear all mandatory equipment including: Neck Guards & Full Cages / Visors in compliance with the rule.

I HAVE READ THIS RELEASE OF LIABILITY AND ASSUMPTION OF RISKS AGREEMENT, FULLY UNDERSTAND ITS TERMS, UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT, AND SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.

By signing the release form, I certify that, as player/parent /guardian with legal responsibility for this participant, I do consent and agree to his/her release as provided above of all releasees and, for myself, my heirs, assigns, and next of kin, I release and agree to indemnify the releases from any and all liabilities incident to my child’s involvement in this tournament as provided above, even if arising from the negligence of the releases, to the fullest extent permitted by the law.
